If you wish for someone else (i.e a relative) to be able to discuss your medical records, including being given access to your test results & appointment information you will need to complete a consent form for this.

You can either collect a form from reception; complete sign and return it to us or you can download the document below; print, complete, sign and send in to us so that we can update your records and discuss any information from your medical records with a third party.

The form also contains your contact preferences, carer details (if applicable) and details of your Next of Kin for completion to help us provide the best possible care for you.

Next of Kin

Your medical next of kin is someone you nominate to receive information about your medical care. If you have not chosen a next of kin, it will usually be assumed to be a close blood relative, spouse or civil partner. They will be kept informed about your care.
